What do we make of this form? Could it be a form to inform the treasury of our demand and redemption status??

This is just a form for the direct disbursement of US securities, the need for which usually results from the execution of a will. There is a similar form for direct disbursement or transfer of shares of a public corporation held in a share account at the issuer (usually Bank of New York Mellon, aka Mellon Bank, which tracks share ownership of hundreds of publicly traded companies). It has nothing to do with lawful money. The issuers of these various Treasury securities are branches of the Treasury Department.
